MyChart - Login Page (2024)



MyChart is a web-based version of our electronic medical information system. If you choose to obtain MyChart access, you will have a secure way to access portions of your medical information, request a prescription renewal, provide information about your health history and family health history, view select laboratory and radiology results, schedule appointments, and communicate with your physician on-line for non-emergency matters. MyChart is meant to provide you with a means to access helpful information like those items listed above, as well as allergy, prescription, and immunization records.

You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, location services and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Ballad Health have control over the contents of these services, and neither Epic nor Ballad Health accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.

MyChart is not your actual medical record and it is not a mirror copy of what Ballad Health holds in your electronic medical record. Even subject areas that are generally accessible may have limitations. For instance, some notes, labs, and radiology results may not go to MyChart at all.MyChart is not a link to your provider's phone, pager, or a means to obtain an immediate answer to a question. If you ask a question through MyChart, you may have to wait a few days to get a response. NEVER USE MyChart FOR EMERGENCY ISSUES OR QUESTIONS!

MyChart is for adult patients 18 and over, or emancipated minors, who are patients of Ballad Health and/or its affiliates who have available records within the Epic medical record system. Parents or guardians of children ages 1-17 may request proxy access to their child's MyChart record, but for minors between the age of 12-17, MyChart functionality will be limited to obtaining immunization and allergy records, providing information about health and family history, and engaging in secure patient message exchanges with physicians regarding care for the minor that does not infringe on the privacy rights that the law gives to them. To request activation of a MyChart account you must be 18 or older.

Proxy access is also available for adult users of MyChart. For example, an older parent may allow their adult child, or spouse, access to their MyChart account.

Absolutely not. Signing up for MyChart is a voluntary service and has no effect on your treatment or ability to seek care from us. We recommend that you sign up though, because MyChart has many features to assist you and your medical team.

Your MyChart e-mail may go directly to your provider or to a pool of nurses or office staff. If your provider is out of the office, your message may be answered by someone who is covering for the provider. Information provided through MyChart will also become a part of your patient medical record and will be accessible to current and future Ballad Health providers and may be made available to authorized third parties who have requested your record.

You will get an e-mail message when there is new medical information to be viewed on MyChart. This means that anyone with access to your e-mail will be able to see the message from MyChart. Although no private medical information will be sent via e-mail, you may not want others to know that you even have new medical information available, so think about what e-mail address you want to provide when signing up to obtain MyChart access - and remember to keep your e-mail address up to date in your MyChart profile.

It may take 2-3 business days before someone can get back to you. MyChart e-mail should never be used for any urgent matter. So if you need an appointment right away or are out of medication and need a refill immediately, do not use MyChart. Use good judgement - and do not send information about someone else’s symptoms or problems because that information will then be tied to your medical record.

MyChart access is provided as a convenience to you, but if you abuse the messaging feature or our staff, you can lose your privilege to access MyChart. Ballad Health may restrict or terminate your access to MyChart at any time for any reason. Sometimes, we may need to restrict access to do maintenance or provide upgrades to the MyChart system. If that is the case, we will try to let you know in advance and also let you know how long it will be unavailable. If you violate the terms and conditions herein, however, we will terminate your access to MyChart.

If you find an error in your MyChart pages, please let us know so we can address it. We try to avoid them, but we are only human and appreciate your help.

Your privacy is really important to us and we maintain the same degree of confidentiality for medical information stored in MyChart that we do for medical information stored in your medical records. We are committed to protecting the confidentiality of your medical information, so we limit our employees' access and ability to view your information based upon their job duties and need to know such information. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We have taken steps to keep all information received electronically as secure as possible against unauthorized access and use.

To ensure the best level of protection for your information, we also recommend that you take certain steps. You may need to upgrade your browser. To view your medical information on MyChart, you need to access it with a Secure Sockets Layer (SSL) compatible browser or terminal (Safari, Firefox 10 or Internet Explorer versions 7.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available.

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Ballad Health by viewing the digital certificate. This certificate verifies the connection between our server's public key and the server's identification.

If you are using a public computer, be sure to close the browser you were using and make sure you are not “saving” your password on the page.

User names and passwords provide two layers of security and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, you can help ensure that your medical information stays private by keeping your login ID and password confidential and changing your password on a regular basis. We recommend that you NEVER share your MyChart login ID and password. By sharing such information, you bear the risk that your confidential information may become known by any number of persons you would not want to have it. By agreeing to MyChart terms and conditions, you agree to bear all liability for any damages or harm that may arise from disclosing your MyChart ID and password to anyone else.

When you sign up for text messages from Ballad Health and its Affiliated Covered Entities, you are signing up to receive text messages related to your relationship with Ballad Health and Affiliated Covered Entities, including updates related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management.

You can opt-out of SMS messages by texting STOP to respective short code. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from. If you want to join again, sign up using Ballad Health MyChart or text HELP to the short code for instructions.

Please note that by signing up for text messages, you acknowledge that messages from the Ballad Health text program, which may include some limited protected health information, will be sent to the mobile phone number you provide, which may be viewed by whomever has access to that mobile phone or the carrier account. Messages sent as part of the Ballad Health text program are unencrypted. Unencrypted messages may be intercepted or received by unintended third parties, and/or stored or archived by service providers and system operators.

Your MyChart account can be deactivated by calling MyChart Nurse Connect at 866-517-5873. Ballad Health also reserves the right to deactivate your account if your use of MyChart violates our Terms and Conditions.

I agree not to hold Ballad Health and/or its affiliates liable for any loss, injury or claims of any kind resulting from my disclosure of the confidential password or confidential information on MyChart. I understand that my activities on MyChart may be tracked by computer audit and that the entries I make may become part of the electronic record. I understand that access to MyChart is a convenience and that Ballad Health and/or its affiliates have the right to deactivate access to MyChart at any time for any reason or for no reason without notice to me.

I understand that use of MyChart is voluntary and I am not required to use MyChart or to authorize a MyChart proxy. I understand that MyChart may not be available to me all the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of Ballad Health and its affiliates. Access is provided on an "as-is, as available" basis and there is no guarantee that I will be able to access MyChart at any particular time.
